If I am the homeowner performing the work myself do I need to submit insurances with my permit application?

Yes.  A Certificate of Exemption (CE-200) is required for any homeowner's doing work themselves.  This is an exemption of insurance because you are not required to carry Worker's Compensation and Disability insurances like a contractor does.  NYS requires us to have this certificate for any building permits issued.
